Please read each question and answer all parts carefully. If you skip information we ask for in the questions, your request may not be considered.The questions we ask are critical to be answered as truthfully and detailed as possible.

I hereby certify that I am the rightful owner/keeper/caretaker/custodian of the animal(s) who is/are the subject of this Animal Surrender Form, hereinafter referred to as “the animal.” I hereby surrender any and all property rights to the animal. I certify that no other person has a right of property to the animal. I understand that by surrendering my property rights to the animal, the animal may be transferred into the custody of an animal shelter. I also hereby certify that the animal has/has not bitten or scratched a human or another animal within the past 10 days. I understand that once I relinquish the animal, the animal will not be available to be returned. I further certify that I have read and understand the terms of this Animal Surrender Form.

Animal Surrender Terms and Conditions

By signing this form, I acknowledge and agree to the following terms and conditions regarding the surrender of my animal: 

1. Voluntary Surrender: I understand that I am voluntarily surrendering my animal to [Maplelatte Animal Shelter Society]. I confirm that this decision is made without any coercion or pressure.

2. Ownership: I certify that I am the legal owner of the animal being surrendered and have the authority to make this decision. I agree to relinquish all rights and ownership of the animal upon surrender.

3. Health and Behavior Disclosure: I acknowledge that I have provided accurate and complete information regarding the animal’s health and behavioral history. I understand that withholding information may affect the animal's placement.

4. No Guarantees: I understand that [Maplelatte Animal Shelter Society] cannot guarantee the placement of my animal in a new home or the outcome of their care and rehabilitation.

5. Release of Liability: I release [Maplelatte Animal Shelter Society], its staff, and volunteers from any liability for any damages or injuries that may occur as a result of the animal’s behavior or health issues.

6. Fees: I understand that there may be surrender fees associated with this process. I agree to pay any applicable fees at the time of surrender.

   Fixed Canine Surrender Fee is $215 each

   Fixed Feline Surrender Fee is $115 each

   Unfixed animal: $400 each
   

7. Reconsideration Period:

   The Surrendering Party has three (3) calendar days from the date of surrender to reclaim the Animal, provided the Animal has not already been adopted, transferred, or otherwise lawfully disposed of.

   
   

   During this period, the Organization is not obligated to hold the Animal exclusively unless prior written arrangements have been made.

8. Payment: I acknowledge and agree that once the full payment has been made, it is stickly non-refundable under any circumstance.

9. Follow-Up: I may be contacted in the future regarding the animal's status or for follow-up purposes, unless I specify otherwise.

10. Final Decision: I understand that once the surrender is accepted, it is final, and I will not have the right to reclaim the animal.
    

Three days after the reconsideration period ends, you may no longer contact the shelter with questions about the pet(s) you surrendered. We reserve the right to block communication if attempts are made to interfere with our decisions**
