I have read the above information carefully and have filled out this application honestly. I understand that omission of information and/or failure to answer all the questions can result in this application being declined.
Any personal information collected is only used by [Maplelatte Animal Shelter Society] for the purpose defined at the time of the collection or a use that complies with these purposes. We do not share your information with any third parties.
Media Consent
By adopting from our shelter, you consent to the use of photos, videos, or stories of you and your adopted pet for promotional, educational, and fundraising purposes. These materials may appear on our website, social media, or printed media. No compensation will be provided, and all content remains the property of the shelter.
On-Site Interview / Instant Adoption
"Instant adoption" means that, if approved after quick interview, the Adopter may take the animal home the same day, once this Agreement and any required paperwork and fee/donations are completed
The Shelter reserves the right to approve or decline any adoption at its sole discretion, at any time during the process.
VACCINATIONS/DEWORMING
[Maplelatte Animal Shelter Society] provides vaccinations for all animals due while in our care. The adoptive family is responsible for vaccinations and continued deworming that come due after the adoption is completed, which can include on-going treatment for minor ailments such as worms.
IF YOU HAVE DOGS AND/OR CATS
We strongly suggest slow introductions if you choose to welcome a new animal coming into your home.
ADOPTION FEE
Most pets adopted through Instant Adoption or On-Site Interview are NOT YET spayed/neutered, so the adoption fee is lower than our standard fee.
NO SURRENDERING
As long as the adopted pet’s microchip is registered with [Maplelatte Animal Shelter Society], the pet must be returned to us if you are no longer able to care for it.
If the pet’s microchip is NOT registered to the Society, you will be responsible for arranging a safe and responsible new home or adoption on your own.
